I have received your latest bill regarding account number ___________.

I regret that my account is now past due. Though I have made every effort to keep my account current, I now find that I am unable to make full payments as originally agreed due to ____________. (Catastrophic illness, death in the family, lost job etc…)

However, I am certain that my financial situation will improve somewhat in the near future due to __________. (Recovering from illness, getting new job, launching new career)

In the meantime, I’m enclosing a token payment of $_____ to show my good intentions.

This tactic will often get you some time to regroup and repair your financial situation so you can resume normal payments.
